For children aged between 12 and 15, a declaration that the child has no criminal record is required by the parent.
• Notarized colored copy of the driver's license
• Notarized colored copies of all identification documents or cards
• Notarized colored copies of all military service documents and discharge papers, if applicable (male applicant aged 18 and over)
• 10-12 original passport-sized photos
• Proof of residential address (notarized copy of a rental agreement or original utility bill)
• Notarized copies of university/college certificates
• Notarized Affidavit of Support for each dependent aged 18 and over included in the application, except for the spouse
